My personal favorite is Katie Couric discussing Time's Person of the Year cover:

Couric: "Tell me why you all decided to honor the American soldier? Wondering why there's no woman on the cover, too?"
Time's Jim Kelly, pointing to cover: "This is a woman."
Couric: "Oh, there you go, oh sorry....I couldn't tell because of her helmet."

Taking 40+ days of vacation off before September 11th and not even sitting down and discussing the August 6th PDB and thinking, "Hey, maybe we should heavily reinforce cockpit doors to avoid anything like that shit Tom Clancy wrote two best-sellers about years ago" would have been a good start... oh, but I forgot, there was no "actionable" intelligence...well, at least Terrorism was Always Priority #1 at the Bush White House...right behind this useless project that Clinton had the sense to balk at...
